The Synergy Choice Bonus 5 is a Single Premium Deferred Fixed Index Annuity with a market value adjustment. The minimum single premium is $25,000. 10% of the accumulated value is available after the first contract anniversary without a withdrawal charge or market value adjustment. Surrender charges are 5 years in duration (may vary by states) and are waived in the event of death, confinement, or diagnosis of a terminal illness. There is a bailout feature available. Withdrawals and taxable distributions may be subject to ordinary income tax and if made prior to age 59 1/2, may also be subject to a 10% federal income tax penalty.
