The Voya Secure Index Outlook is a single premium deferred fixed index annuity. The minimum initial contribution is $15,000 for qualified and non-qualified funds. 10% of the accumulated value may be withdrawn annually beginning in year one. Systematic Withdrawal is an option. There are ten years of surrender charges (10,9,8,7,6,5,4,3,2,1) which may be waived in the event of death, entrance into a nursing home or hospital, or diagnosis of a terminal illness. The IncomeProtector Withdrawal Benefit is an optional rider which, if elected for an additional cost, guarantees an annual withdrawal amount up to a percentage of the Minimum Guaranteed Withdrawal Benefit Base (MGWB Base) per year. Available during the first policy year, payments are guaranteed for the life of the annuitant. Owner and annuitant must be between 50 and 80 years of age at issue. Withdrawals may result in federal income tax and an additional 10% IRS penalty tax prior to age 59 ½. All features or contract may not be available in all states.
