The Keystone Index 5 is a single premium deferred index annuity. The minimum single premium is $10,000 for non-qualified and qualified money ($5,000 per strategy). 10% of the premium paid may be withdrawn beginning in the first contract year and up to 10% of the annuity value be withdrawn beginning after the first contract year. Systematic withdrawal is an option if funds are allocated to the fixed strategy (lump sum only on index strategies). Surrender charges are 5 years in duration (may vary with state availability); and are waived in the event of annuitization after the fifth policy year, death, entrance into a nursing home (up to 25% of annuity value) or hospital (up to 25% of annuity value), or diagnosis of a terminal illness. State variations may apply. Withdrawals and taxable distributions may be subject to ordinary income tax and if made prior to age 59 1/2, may also be subject to a 10% federal income tax penalty.
