The BalancedChoice Annuity 10 is a single premium deferred index annuity with a market value adjustment (MVA). The minimum single premium is $5,000 for qualified and non-qualified money. 5% of the accumulation value is available during the first 12 months and 10% of the accumulation value each 12 month period thereafter. Surrender charges are ten years in duration (9.6,9.2,8.2,7.2,6.2,5.2,4.2,3.1,2.1,1.0) and are waived in the event of annuitization after the first contract year, death, confinement, or diagnosis of a terminal illness. There is an optional lifetime income rider which, if elected for an additional cost, provides an option to receive lifetime income withdrawals without incurring withdrawal charges or MVA. There is an optional death benefit rider which, if elected at an additional cost, offers a death benefit which guarantees the amount paid to the beneficiaries will be the higher of the contract’s death benefit or enhanced death benefit amount. The Step-Up Rider is offered with the death benefit rider at an additional cost and adds a bonus to the balanced allocation value. Withdrawals and taxable distributions may be subject to ordinary income tax and if made prior to age 59 1/2, may also be subject to a 10% federal income tax penalty.
