The ChoiceFour is a Single Premium Deferred Annuity.  The initial minimum contribution is $10,000 qualified and non-qualified.  Additional contributions are allowed within the first policy year only.  Withdrawal of interest earnings is available.  Surrender charges vary by state chosen and are waived in the event of annuitization, death, or entrance into a nursing home.  There are four contract options available:  Base contract (9-year surrender charge schedule, free withdrawals of prior 12 months interest), MVA option 9-year surrender schedule, 1.50% premium bonus, MVA, free withdrawals of prior 12 months interest), Liquidity Option (6-year surrender schedule, free withdrawals of 10% of accumulation value annually after year two), and combination of MVA and Liquidity Option (6-year surrender schedule, 1.50% premium bonus, MVA, free withdrawals of 10% of accumulation value annually after year two).  Withdrawals may be subject to Federal/State income tax, and if taken prior to age 59 1/2, a 10% Federal Penalty tax.  Guarantees based on claims paying ability of insurer.
